The Forum is now accepting registrations for the 2011-2012 Forum Adult Hockey League season. The 2011-2012 season details are as follows:

Starting September 18th through March 11th
Cost: Players - $500.00 Goalies - $250.00

Registration form must be turned into Adrian Render by August 20th 2011

- 22 REGULAR SEASON GAMES
- 3 PLAY-OFF GAMES
- ALL USA HOCKEY RULES APPLY
- GAME JERSEYS SUPPLIED
- (3) 17 MINUTE STOP TIME PERIODS
- 13 SKATERS MINIMUM PLUS GOALTENDER ON ALL ROSTERS.

ALL PLAYERS MUST BE REGISTERED AND PAID IN FULL BEFORE OR BY SEPTEMBER 18TH!

You must complete the USA Hockey Consent to Treat, USA Hockey Waiver of Liability, and FAHL League Registration Forms, and annually be registered with USA Hockey. Printouts of all four forms must be provided by mail (address can be found on contacts page) or in person in order to complete your registration.
